This list may not include all of our in or out-of-network insurance providers. Please check with your health insurance provider to confirm network status and coverage of our services. Our staff does not have access to the details of your health benefit plan and coverage.

*This plan is discount only and does not make the insurance carrier part of the contracted network. Plan holders will receive in-network rates, but services will be applied to out-of-network benefits.
*Please check in-network status for our providers by clinic NPI (1861412785) OR your plan ID, as all plans are not in network (i.e. Humana Gold Plus)
*Commercial Health Plan only, effective 4/1/2024
*Quartz Community Plan is an out-of-network plan for our clinic. The new Quartz Cigna PPO Plan will not cover care at Associated Physicians.
*As of 1/1/2024, Security Health Plan Medicare Advantage Plans no longer have out-of-network benefits for DME supplies

Té un pla EPO?
EPO significa el pla "Organització de proveïdors exclusius". Com a membre d’una EPO, podeu utilitzar els metges i hospitals de la xarxa EPO, però no podeu sortir de la xarxa per obtenir assistència. No hi ha avantatges fora de la xarxa, cosa que significa que si el pacient va a un proveïdor que no és EPO, els seus serveis quedaran fora de la seva butxaca.
Tant WPS com Alliance tenen plans EPO en què els metges associats no participen, així que assegureu-vos de consultar els proveïdors de la xarxa abans de seleccionar-lo.
