This list may not include all of our in or out-of-network insurance providers. Please check with your health insurance provider to confirm network status and coverage of our services. Our staff does not have access to the details of your health benefit plan and coverage.

*This plan is discount only and does not make the insurance carrier part of the contracted network. Plan holders will receive in-network rates, but services will be applied to out-of-network benefits.

*Quartz Community Plan is an out-of-network plan for our clinic. The new Quartz Cigna PPO Plan will not cover care at Associated Physicians.

EPO sta per piano "Exclusive Provider Organization". In qualità di membro di un EPO, puoi utilizzare i medici e gli ospedali all'interno della rete EPO ma non puoi uscire dalla rete per le cure. Non ci sono vantaggi fuori dalla rete, il che significa che se il paziente si rivolge a un fornitore non EPO, i suoi servizi saranno in tasca.
Sia WPS che Alliance hanno piani EPO a cui i medici associati non partecipano, quindi assicurati di controllare i tuoi fornitori in rete prima di selezionare il tuo medico.
