This list may not include all of our in or out-of-network insurance providers. Please check with your health insurance provider to confirm network status and coverage of our services. Our staff does not have access to the details of your health benefit plan and coverage.

*This plan is discount only and does not make the insurance carrier part of the contracted network. Plan holders will receive in-network rates, but services will be applied to out-of-network benefits.
*Please check in-network status for our providers by clinic NPI (1861412785) OR your plan ID, as all plans are not in network (i.e. Humana Gold Plus)
*Commercial Health Plan only, effective 4/1/2024
*Quartz Community Plan is an out-of-network plan for our clinic. The new Quartz Cigna PPO Plan will not cover care at Associated Physicians.

Adakah anda mempunyai rancangan EPO?
EPO bermaksud rancangan "Organisasi Penyedia Eksklusif". Sebagai ahli EPO, anda boleh menggunakan doktor dan hospital dalam rangkaian EPO tetapi tidak boleh keluar dari rangkaian untuk mendapatkan rawatan. Tidak ada faedah di luar rangkaian yang bermaksud jika pesakit pergi ke penyedia bukan EPO, perkhidmatannya akan habis.
WPS dan Alliance kedua-duanya mempunyai rancangan EPO yang tidak disertainya oleh Pakar Perubatan Bersekutu, jadi pastikan anda memeriksa penyedia rangkaian anda sebelum memilih doktor anda.
