This list may not include all of our in or out-of-network insurance providers. Please check with your health insurance provider to confirm network status and coverage of our services. Our staff does not have access to the details of your health benefit plan and coverage.

*This plan is discount only and does not make the insurance carrier part of the contracted network. Plan holders will receive in-network rates, but services will be applied to out-of-network benefits.

*Commercial Health Plan only, effective 4/1/2024
*Quartz Community Plan is an out-of-network plan for our clinic. The new Quartz Cigna PPO Plan will not cover care at Associated Physicians.

Je! Unayo mpango wa EPO?
EPO inasimama kwa mpango wa "Shirika la Watoa Huduma Maalum". Kama mwanachama wa EPO, unaweza kutumia madaktari na hospitali ndani ya mtandao wa EPO lakini huwezi kwenda nje ya mtandao kupata huduma. Hakuna faida za nje ya mtandao maana ikiwa mgonjwa huenda kwa mtoa huduma ambaye sio EPO, huduma zake zitakuwa nje ya mfukoni.
WPS na Alliance zote zina mipango ya EPO ambayo Waganga Wanaoshiriki haishiriki kwa hivyo tafadhali hakikisha kuwa unaangalia watoa huduma wako wa mtandao kabla ya kuchagua daktari wako.
